Discharge plan, in plain words

It is the hospital's plan for how you or your parent will manage safely after going home.

What is a discharge plan?

A discharge plan is what the hospital team prepares before a patient goes home. It usually covers medicines, wound or equipment care, follow-up appointments, what the patient can and cannot do, and who to call with questions.

It may be a printed sheet, a summary letter or a talk with a nurse or therapist. Ask for it in writing.

Why does it matter for a patient or family?

The first days at home carry the most risk. The plan is the link between the ward and the house, so that the family is not guessing how to transfer, bathe or climb stairs.

Gaps in the plan, such as no walking aid or no instructions for the stairs, are worth raising before leaving, not after.

What should we ask before discharge?

  • What can the patient do alone, and what needs help?
  • Do they need a walking aid, commode or other equipment?
  • Which exercises should they do, and who will review them?
  • When are the follow-up appointments, and who do we call if something worries us?
  • Is rehabilitation arranged, and is a home visit possible?

An everyday example in Ipoh

A family collects their mother from a hospital in Ipoh after a hip operation. They ask the ward staff about the steps at the house entrance, where the bedroom is, and whether she should use a frame or a stick.

They take a copy of the discharge summary, and share it with the home physiotherapist before the first visit, so the plan on her own stairs matches what the surgeon advised.

Use our after hospital discharge checklist and questions to ask before discharge. Read about continuing hospital physio at home, going home after stroke and post-surgical rehabilitation.

Questions people ask

Who writes the discharge plan?

The hospital team writes it, with input from doctors, nurses and therapists. If you did not get one, ask the ward staff or the clinic where the patient is followed up for a written summary and instructions.

Can physiotherapy at home start right after discharge?

In most cases yes, once the surgeon or doctor allows it. Share the discharge papers and any rules, such as weight limits or movements to avoid, so the physiotherapist can plan safely from the first visit.

What if the plan does not match what we see at home?

Tell the hospital team or your doctor as soon as possible. If the house layout, the patient's condition or the equipment does not match the plan, adjustments are better made early. A home physiotherapist can also report what they see.
