Continuing hospital physio at home

The ward physiotherapist got you standing, and now you wonder who keeps it going.

Why does physiotherapy need to carry on after the ward?

Because the ward gets you safe, and home is where you get strong. Hospital stays are short, and the work that matters for walking, stairs and daily tasks mostly happens afterwards.

The World Health Organization (WHO) says stroke rehabilitation should start once the patient is medically stable, ideally within the first few days, and involve a team including physiotherapy [1]. The same idea holds beyond stroke: begin in hospital, then continue.

What did the ward actually teach me?

Write it down before you forget. Ask the ward physiotherapist, or the family member who was present, to list:

  • How you get in and out of bed.
  • How you stand up, and which hand or leg leads.
  • Which walking aid you use and how.
  • Which exercises to do, and the number of repeats each day.
  • Which movements to avoid.

A short phone video of each exercise beats a sheet. If you did not get either, ask at your next review.

How do ward exercises change at home?

The ward has a flat floor and a bed with rails. Your home has a threshold, a first-floor bedroom and a wet bathroom.

Common Ipoh set-ups need their own plan: a front step into a terrace house, a staircase with a turn, a low squat toilet. A physiotherapist can practise these with you and change the exercises to match.

How do I hand over to a home physiotherapist?

Give them the discharge summary, the exercise sheet or videos, the date of your next review and anything the doctor said to avoid. Then tell them honestly what you can do now.

We begin with a short assessment of transfers, walking, balance and the stairs. Our page on after hospital discharge in Ipoh lists what to gather.

What if I am waiting for the hospital’s own physio?

Outpatient slots can take time, and weeks of inactivity weaken muscles. Home physiotherapy can bridge that gap if your doctor agrees.

Read waiting for a hospital physio appointment for what to ask and how to keep the place in the queue.

What about stroke?

Rehabilitation after stroke carries on at home with daily practice, not only visits. We work from what the stroke team taught and set small, clear goals. Read going home after a stroke ward in Ipoh for what families do first.

Our stroke rehabilitation at home page explains the approach.

How do we help?

Home visits across Ipoh are available now through our home physiotherapy service. Our Canning Garden centre, near Fatimah Hospital, is opening soon. Prices differ between home and centre, and we quote on WhatsApp.

Questions people ask

Can I just repeat the ward exercises at home?

Yes, at the pace you were given, and no more, until a physiotherapist reviews you. Ward exercises are a starting point. A home visit checks that you do them safely, and moves them on when your body is ready.

What if I did not see a physiotherapist in hospital?

That happens, especially after a short stay. Tell us what you can and cannot do, and share the discharge summary. We start with a gentle assessment of walking, transfers and stairs, within your doctor's advice.

How do I get the hospital exercises?

Ask the ward physiotherapist for a written sheet or a short video on your phone before you leave. If you did not, ask at your first review or send the sheet your family took photos of to the home physiotherapist.

Should hospital and home physiotherapy overlap?

They can run side by side. Tell each about the other so exercises are not doubled or contradicted. Your doctor's review always comes first, and the home physiotherapist adjusts to what the hospital advises.

How does this work after a stroke?

After stroke, rehabilitation works best when it carries on after the ward. We pick up from what the stroke team taught, set simple daily goals at home and involve the family so practice happens between visits.
