What are the three stages of a frozen shoulder?
A frozen shoulder (adhesive capsulitis) usually moves through three stages: freezing, frozen and thawing. Pain leads, then stiffness, then slow recovery.
Knowing the stage matters, because what helps in one stage can hurt in another. Recovery is measured in months, and it varies from person to person.
What is the freezing stage like?
The shoulder starts to ache, and it is worse at night. You may wake when you roll onto it, and ordinary movements, such as putting on a shirt, begin to hurt.
The joint is irritated, so the aim is to calm it. Avoid pushing through sharp pain. Gentle movement within comfort, such as letting the arm swing loosely while you lean forward, keeps the joint from stiffening further.
Our page on shoulder pain at night has sleeping positions that help.
What is the frozen stage like?
Pain starts to ease, but movement is now limited. You cannot reach behind your back, lift your arm to comb your hair or reach the top shelf of the kitchen cabinet.
Now the joint is stiff rather than angry. A physiotherapist can introduce firmer stretches and mobility work, and you do short sessions at home each day. Regular, gentle effort works better than one painful session.
What is the thawing stage like?
Movement slowly returns. The arm reaches higher, and the night pain fades. This is the time to rebuild strength, because the muscles around the shoulder have weakened from months of protecting it.
Add light resistance and everyday practice: carrying shopping, lifting a rice cooker, hanging washing. Do not rush back to heavy lifting or a hard badminton smash.
What do injections and exercise add?
A 2020 review of 65 frozen shoulder studies found corticosteroid injection was the only treatment clearly better than physiotherapy in the short term [1]. Adding a home exercise programme gave extra benefit later [1].
The injection may calm pain for a while, and the exercise keeps the movement you gain. Your doctor decides if an injection suits you. Read more in injection or physiotherapy.
How can I manage daily tasks in Ipoh homes?
Small changes make life easier while the shoulder recovers.
- Hang laundry at chest height and move the bar down if you can.
- Store everyday items on the middle shelves, not the top shelf.
- Dress the painful arm first, and undress it last.
- Adjust the seatbelt before moving off, using the good arm.
- Avoid carrying heavy bags on the stiff side, including market shopping.
If you ride a motorbike, ask whether it is safe to do so while the shoulder is weak. Grip strength and the reach to the handlebar matter.
When should I see a doctor first?
See a doctor if the shoulder hurts after a fall, looks deformed, is hot and swollen, or if you have chest pain, breathlessness or pain spreading down the arm. Those signs are not typical of a frozen shoulder.
If you have diabetes, mention the shoulder to your doctor. Hospital Raja Permaisuri Bainun, Fatimah Hospital and the private hospitals in Ipoh can all assess it.
How can a physiotherapist help at home?
We check which stage you are in, how far the joint moves and which tasks matter most to you. Then we choose the right mix of gentle movement, stretching and strengthening.
