Knee pain in older women in Perak

Your knees ache at the wet market, on the stairs and on the prayer mat or kitchen floor.

Why do older women’s knees hurt so much?

Knees carry body weight every day, and wear and tear in the joint (osteoarthritis) builds up slowly. After years of stairs, carrying shopping and children, standing in kitchens and sitting on floors, the joint may protest.

In a Greater Kuala Lumpur study of adults aged 55 and over, one in three had knee pain [1]. That was in the Klang Valley, not Perak, but it shows how common the problem is. You are not exaggerating.

Which everyday positions put a load on the knee in Ipoh?

Ipoh homes have squat toilets, floor-level dining, prayer or altar kneeling, and steep terrace-house stairs. The wet market means long walks on uneven, damp floors.

Each is fine for a healthy knee, but a sore knee takes strain at deep bends. If one of these is the daily sore spot, change it first. See squat toilets and knee pain and sitting on the floor with knee pain.

What small changes can help this week?

  • A raised toilet seat or a chair-height toilet where possible.
  • A firm chair at the dining table instead of the floor.
  • A rail on the stairs, and one trip up and down instead of repeated trips.
  • Supportive, non-slip shoes for the market, not thin slippers.
  • A trolley bag instead of carrying shopping.

These are kind to the knee and keep her independent. Our page on knee pain on stairs covers more.

What does the evidence say about exercise?

A Cochrane review of 54 trials found that land-based exercise for knee osteoarthritis reduced pain by about 12 points on a 0 to 100 scale. It also improved physical function by about 10 points [2]. In plain words, gentle strengthening and movement ease the knee for people who stick with it.

The World Health Organization (WHO) lists exercise, weight management and education as the main management, and says glucosamine and viscosupplementation are not considered effective [3]. Ask your doctor before paying for supplements.

How do I help my mother without taking over?

Offer to take the heaviest jobs, such as carrying the bags from the car, but keep her involved in what she enjoys. A mother who is told to sit still can lose confidence and strength.

Ask: “Which job hurts most? Let me do that one, and you do the rest.” Dignity matters. If she hesitates, a physiotherapist’s voice can help.

How can a physiotherapist help at home?

We watch her walk, climb, sit and rise, and test the knee. Then we build a plan for her stairs, her toilet and her kitchen, with exercises to strengthen the thigh and hip and a way to pace the day.

Questions people ask

Is knee pain just part of getting old?

Age plays a part, but pain is not a fixed price you pay. Wear and tear in the knee (osteoarthritis) responds to exercise, weight management and sensible changes in daily positions. Ask a doctor to confirm the cause, because other conditions can feel similar.

Should I stop squatting, kneeling and sitting on the floor?

Not necessarily forever, but reduce them while the knee is sore. Use a toilet seat riser, a chair at mealtimes and a prayer chair if kneeling hurts. Ask your physiotherapist when and how to return gradually.

Do glucosamine and joint supplements help?

The World Health Organization (WHO) says glucosamine and some injections are not considered effective for osteoarthritis. Exercise, weight management and education are the main tools. Talk to your doctor before spending money on supplements.

When do I need a doctor, not just exercise?

See a doctor for a hot, red, swollen knee, fever, sudden severe pain after a fall, inability to bear weight, or a knee that locks or gives way. Persistent night pain also deserves a check.

Can a physiotherapist visit my mother at home?

Yes. A home visit looks at the squat toilet, the bed height, the stairs and the kitchen, and builds a gentle plan for those places. We can include the daughter or helper so exercises continue between visits.
