What is carpal tunnel syndrome?
Carpal tunnel syndrome is pressure on the median nerve as it passes through a narrow channel in the wrist, called the carpal tunnel. The nerve supplies feeling to the thumb, index finger, middle finger and half of the ring finger, so those are the fingers that tingle or go numb.
It can also weaken the grip. Our overview of wrist and hand pain shows how it sits among other hand problems.
What does it feel like in everyday Ipoh life?
Most people notice night tingling first. You wake with a numb hand, shake it, and the feeling drifts back. In the day it can show up while holding a phone, gripping a steering wheel, riding a motorbike or using a mouse.
Over time you may drop cups, fumble with buttons or find it hard to open jars. Our blog on numb hands at night walks through what to note before a visit.
Sleeping with the hand curled under the pillow, or the wrists bent while sleeping under a fan, can make it worse.
Who is it most likely to affect?
It is more common in women and in people in mid-life. Pregnancy, diabetes, an underactive thyroid and some types of arthritis raise the chance, because they change fluid balance or nerve health.
Repeated wrist work such as typing, sewing, assembly work and tool use can add to it. If you have diabetes, our blog on diabetes and exercise in Perak offers useful context.
What does a physiotherapist check first?
We ask when the symptoms come, which fingers are affected and what eases them. Then we test sensation, strength and a few simple movements that squeeze or stretch the nerve at the wrist.
We check:
- feeling in each finger, and whether the little finger is spared.
- thumb strength and any thinning of the muscle at the base of the thumb.
- how the wrist, elbow and neck move, since nerve pressure can arise at other places.
- grip and pinch strength compared with the other hand.
- your sleeping position, desk, phone use and tool grip.
If the pattern points to the neck, the elbow or a different nerve, we explain and guide you.
What helps carpal tunnel syndrome?
For mild and moderate cases, a combined plan is the usual start. It reduces pressure on the nerve at night and in the day, and keeps the nerve moving.
A plan can include:
- a night splint that holds the wrist straight, fitted to you.
- changes to desk, phone and tool use, to avoid long periods of wrist bend.
- nerve glide exercises, which help the nerve slide through the tunnel.
- stretching and strengthening for the hand, forearm and shoulder.
- advice on warm-up and activity pacing.
Progress takes patience. We review it every visit and adjust the plan. If symptoms stay or weakness grows, your doctor may discuss an injection or surgery.
What can I do this week?
Start with small habits that take pressure off the wrist.
- Keep the wrist straight while sleeping. A soft wrist splint from a pharmacy can be a start, and we can advise on fit.
- Avoid sleeping with the arm under the pillow or head.
- Take a short break from typing, gripping or the handlebars every half an hour.
- Hold your phone in the other hand or use voice notes for a few days.
- Keep the hand out from under the fan’s direct breeze if cool air leaves it stiff.
Note when the hand wakes you, and bring that to your assessment.
When should I see a doctor first?
Go to the emergency department if the hand is suddenly white, blue or cold, or if numbness comes with weakness in the face or leg, or trouble speaking. Go too after a wrist injury if the hand is badly swollen or the wrist looks out of shape.
See a doctor if the numbness is constant, the thumb muscle is thinning, your grip is clearly weaker, or symptoms are not easing after a few weeks. Hospital Raja Permaisuri Bainun, Fatimah Hospital and Pantai Hospital Ipoh can assess these.
How do home sessions in Ipoh work?
A physiotherapist comes to your house, fits a splint if it suits you, looks at your bed, desk and phone habits, and teaches exercises you can repeat daily. Home visits are available across Ipoh now, and our Canning Garden centre is opening soon.
