Why do joints get stiff?
A stiff joint is one that does not move as far or as easily as it should. It can feel tight, creaky or slow to “start” after you have been still.
The cause can be wear and tear in the joint (osteoarthritis), an old injury, swelling from arthritis, weak surrounding muscles, or simply too much sitting. A physiotherapist works out which, because the answer changes the plan.
What does stiffness tell you about the cause?
How long stiffness lasts is a useful clue. It is not a diagnosis, but it helps you decide what to do.
| Pattern | What it may mean | What to do |
|---|---|---|
| Stiff for a few minutes after sitting or waking | Wear and tear or muscle tightness | Gentle movement, then a physiotherapy check |
| Stiff for half an hour or more, with swelling in both hands or feet | Inflammatory arthritis | See a doctor, then plan movement |
| Stiff after an injury or cast | Joint and muscle lost movement while resting | Physiotherapy to restore range |
| Stiff with a hot, red, swollen joint | Flare or infection | Doctor the same day |
This table is a guide only. When in doubt, ask a doctor first.
What can you do this week?
A short routine in the first minutes after waking makes the rest of the morning easier. Try this, within comfort.
- Before getting up, bend and straighten the knees and ankles slowly, ten times.
- Open and close the hands, and roll the wrists.
- Sit on the bed edge and turn the head gently left and right.
- Stand up with support, march on the spot for a minute, then walk to the bathroom.
During the day, stand and move every 30 minutes. A warm shower before exercise loosens the joints, which suits Ipoh’s cooler mornings and rainy afternoons.
Which Ipoh habits stiffen joints?
Long mornings in the kopitiam, low seats on the floor, the car or the cold air-conditioned office all keep joints in one position. Sitting cross-legged on the floor for prayers or a family meal can be especially hard on the knees and hips.
Our guides to sitting on the floor with knee pain and weather and joint pain cover these daily habits.
How does physiotherapy loosen stiff joints?
We find out which joints are stiff and why, then work on movement and strength together. A joint moves better when the muscles around it are strong enough to control it.
- Gentle hands-on techniques and guided movement to widen range.
- Stretches for tight muscles around the joint.
- Strength exercises, since stronger muscles take load off the joint.
- Advice on warming up, pacing and keeping active through the day.
- A home routine matched to your hardest times of day.
For the knee, a Cochrane review found that land-based exercise reduced pain by about 12 points on a 0 to 100 scale immediately after treatment [1]. See also our page on knee osteoarthritis.
When should you see a doctor first?
See a doctor if a joint is suddenly hot, red and swollen, if stiffness lasts half an hour or more most mornings, or if it comes with fever, weight loss or rash.
Go to the emergency department for a joint that is very painful with fever, or if you cannot move it at all after a fall. Our page on when to seek medical care lists more signs.
What does a home visit look like?
The physiotherapist asks which joints stiffen and when, then watches how you stand, walk and use stairs. We check range of movement, strength and how you do things like getting out of a low chair.
You leave the first visit with a short routine to do every day, and we review progress together at the next one. Read more on home physiotherapy in Ipoh.
