Why do I wake with a stiff neck?
A stiff neck on waking is the neck muscles and joints guarding after being held in a strained position for hours. When you try to turn, they tighten to protect you, and it hurts.
Common triggers are a pillow that is too thick, too flat or too soft, and sleeping on your stomach with the head turned. Falling asleep on a sofa, or cold air from an aircon or fan blowing on your neck all night, can do it too. After a stressful week the muscles tighten more easily.
It looks alarming, but it is usually a muscle and joint strain and not something serious. Most episodes ease within a few days with gentle movement.
Who is helped most?
Anyone who has a stiff neck more than once, or one that takes more than a few days to clear. That includes people whose sleep or work is being affected, and those who feel the neck is never quite right.
It also suits people who sleep with the aircon directly on them, who take long flights or bus rides, or who spend long hours at a desk. If neck pain is a regular issue, see our page on neck pain.
What should I do today?
Move gently and stay warm. Keep the neck moving within comfort so it does not set stiff.
- Get out of bed by rolling onto your side and pushing up with your arms, so your neck stays in line.
- Take a warm shower and let the water run over the neck and shoulders.
- Slowly turn your head a little each way, and nod gently, as far as is comfortable.
- Roll your shoulders backward and down a few times.
- Apply a warm towel to the neck for ten minutes, or use a cool pack if that feels better.
Avoid forcing the neck or twisting it fast to “crack” it. Our guide on heat or ice helps you choose.
What do we check if it keeps happening?
If the stiffness returns, we look for the reason. We listen to your sleep, pillow and routine, and then examine how you move.
- How far your neck turns, tilts and bends, and which direction hurts.
- Tight or tender muscles at the neck, shoulders and upper back.
- Posture of the head and shoulders when sitting and standing.
- Strength and feeling in the arms.
- Your pillow, mattress and sleeping position, and the set-up of your desk and phone.
The answer is a combination: a high pillow plus a head-forward desk posture. Fixing both works better than fixing one.
What does treatment look like?
Treatment eases the tight muscles with gentle hands-on techniques, restores neck movement and builds the strength that holds your head comfortably. We then teach exercises and adjust your sleep setup.
We help you pick a pillow that keeps your neck level with your spine when you lie on your side or back. Our guides on choosing a pillow for neck pain and sleeping position explain the basics.
If we find a sign of something beyond a simple strain, we say so and help you see a doctor.
What can I do to stop it coming back?
Small changes at night and by day reduce repeat episodes.
- Sleep on your back or side, with a pillow that fills the gap between your shoulder and head.
- Avoid sleeping on your stomach, and avoid dozing on the sofa with your head tilted.
- Point the aircon or fan away from your head and neck.
- Take a movement break from the screen every half hour.
- Keep up daily walking, as a stroll in an Ipoh park in the cool of the morning loosens the shoulders.
When should I see a doctor first?
See a doctor if the stiffness follows a fall or accident, comes with a fever and severe headache, or does not begin to improve within a few days. Also see one if you have numbness, tingling or weakness in an arm or hand.
Go to the emergency department at once for a sudden severe headache, trouble speaking, a drooping face or chest pain. Our page on when to seek medical care lists more.
