Physio exercises or hands-on treatment?

You expected the physiotherapist to rub the sore spot, and instead you were given exercises.

Why does physio include exercise as well as hands-on work?

Because each does a different job. Hands-on treatment can ease pain and stiffness so you can move; exercise gives the body the strength and control to keep moving well.

Without the first you may struggle to start. Without the second the relief tends to fade.

Most good plans start with the first and move steadily towards the second.

What does the evidence say?

For wear and tear in the knee or hip joint (osteoarthritis), the World Health Organization (WHO) lists exercise, weight management and education. It adds joint replacement for severe damage [1]. Exercise is at the centre.

For long-lasting low back pain, a 2023 WHO guideline recommends education and structured exercise, and also some physical therapies such as manipulation and massage [2]. So both have a place, and exercise is not optional.

What about urut and tuina?

Ipoh families who grew up with urut, tuina or a trusted masseur, and those experiences can bring real comfort. There is no need to choose between them and physiotherapy.

The useful step is to tell your physiotherapist what you have had done. Avoid forceful work on a recent fracture, a fresh surgical scar or a hot swollen joint. Read more in physiotherapy vs urut tradisional and physiotherapy vs massage.

How can I judge the mix in my sessions?

Ask yourself after each visit: do I know what was done, why, and what I should do at home? If the answer is no, ask.

You can say: “Could you explain what the hands-on part is for, and which exercise keeps the benefit?” A good physiotherapist will be glad to.

When do I want more hands-on work, and when more exercise?

Earlier on, when pain is high or a joint is very stiff, hands-on work may take a bigger share. As pain settles, exercise should grow, especially for the knees, hips and back of older adults.

After surgery, your surgeon’s instructions decide what is safe. Read manual therapy and exercise rehabilitation for what we offer.

What would a home session look like with us?

The physiotherapist asks about your pain and day, checks how you move, uses hands-on techniques where they suit, and teaches the exercises on the spot. You practise them in the room where you will actually do them.

Home visits across Ipoh are available now through our home physiotherapy service. Our Canning Garden centre, near Fatimah Hospital, is opening soon. We quote prices on WhatsApp.

Questions people ask

Why does my physiotherapist give me exercises when I wanted massage?

Hands-on work can ease pain for a while, but exercise is what builds strength and confidence so the problem stays settled. Most plans use both: touch to make movement easier, then exercise to make the change last.

Is massage a waste of money?

No. Massage and manual therapy can feel good and help you move more comfortably for a time. They work best as a way into exercise, not as the whole plan. Ask your physiotherapist how they fit together for you.

Can I keep going for urut or tuina alongside physio?

Some families do. Tell your physiotherapist what you have had done, and avoid forceful work on a recent fracture, a new surgical area or a painful swollen joint. If you take blood thinners or have osteoporosis, ask your doctor first.

How much hands-on treatment should a session include?

It depends on the problem and the stage. In the early days after injury there may be more hands-on work. Later the balance moves to exercise. You should always leave knowing what to practise at home.

What if I only want hands-on treatment?

Tell us. We can explain what manual therapy can and cannot do for your condition, and offer it where it suits you. We will also suggest the smallest set of exercises that helps keep the benefit.
