What is an ankle sprain?
An ankle sprain is an overstretch or small tear of the ligaments, the tough bands that hold the ankle bones together. Most sprains are on the outer side of the ankle, after the foot rolls inwards.
The ankle swells, turns tender and sometimes bruises over a few hours. Putting weight on it hurts, and the joint may feel loose or unreliable.
How does it usually happen in Ipoh?
The ground does most of the work. A cracked five-foot-way, a drain edge, a wet tile outside the kitchen, or a missed step off the kerb can roll the foot in a second.
Sport and hiking add more chances. Futsal and badminton land and turn on the ankle, and the roots and loose stones on trails around Bukit Kledang or Gunung Lang catch the foot on the way down. Slippers and high heels give very little support on these surfaces.
Older adults can sprain an ankle on something small, such as the lip of a bathroom door. A sprain in an older person can also be the first sign of poor balance, which is worth looking at. Our page on balance problems explains why.
What do we check at the first visit?
We ask how the foot moved when it happened, how you managed since, and what the ankle must do for you now. Then we test the whole foot and leg.
- Where the swelling and tenderness sit.
- Whether the bone itself is tender, which would point to a fracture.
- How far the ankle moves up, down and side to side.
- How the ligaments feel when gently tested.
- Strength of the calf and the muscles that hold the ankle steady.
- Balance on one leg, with eyes open, then closed.
- How you walk, climb stairs and step off a kerb.
If anything suggests a break or a nerve problem, we tell you and help you get a scan or see a doctor first.
How is an ankle sprain treated?
The first days are about calming the swelling and keeping the ankle moving within comfort. Weeks two onward are about rebuilding strength and balance, which is what prevents the next roll.
A sprain plan usually follows these steps.
- Reduce swelling with elevation and easy movement, and protect the ankle with support if needed.
- Restore the full range of movement, up, down and side to side.
- Strengthen the calf and the muscles around the ankle with bands and body weight.
- Retrain balance, from standing on both feet to one leg, then onto softer or uneven surfaces.
- Add walking outdoors, then jogging, turning and jumping, if your sport needs them.
A guideline in the Journal of Orthopaedic & Sports Physical Therapy (JOSPT) was revised in 2021 and covers first-time sprains and chronic ankle instability [1]. So it speaks to the ankle that keeps rolling as well as the first injury. We use it as a map and fit the plan to you.
What can I do this week?
These small steps keep the ankle moving without punishing it.
- Raise the foot on a cushion when you sit, higher than your hip if you can.
- Draw the alphabet in the air with your toes, a few times, as pain allows.
- Walk short, flat distances, in shoes with a firm sole.
- Avoid slippers on wet floors and uneven paths for now.
- Stand beside the kitchen counter and practise holding your balance on the sore leg, with a hand on the surface.
If the ankle swells more, or an exercise brings a sharp pain, ease off and tell us.
When should I see a doctor first?
Go to a doctor or the emergency department if you cannot put any weight on the foot. Go too if the bone at the back or outer edge of the ankle is tender, or the foot looks out of shape. Also go if the toes turn numb, pale or cold, or the calf becomes swollen and painful.
See a doctor if the ankle is still very painful after a week or the swelling is not settling. Hospital Raja Permaisuri Bainun, Fatimah Hospital and Pantai Hospital Ipoh can see you for these. If a break is found and the ankle later feels stiff, see ankle stiffness after a cast.
How do home sessions in Ipoh work?
A physiotherapist comes to your house, tests your ankle on your own floor and stairs, and checks the shoes you wear every day. Balance training needs only a wall, a chair and a small space.
Visits are available across Ipoh now, and our Canning Garden centre is opening soon. Prices are quoted on WhatsApp after a short description. See how home physiotherapy works, and read our guide to ankle care on Ipoh hill trails.
