Rolled your ankle on an Ipoh trail?

Your foot slid off a wet root and the ankle gave way, far from the car.

What happens when you twist an ankle on a trail?

On a hill trail near Ipoh, a root, a loose stone or a patch of wet clay can turn the foot inward in a second. The ligaments on the outside of the ankle stretch or tear, which is a sprain. Swelling and pain follow, and putting weight on the foot feels unsafe.

The first minutes matter. Stop, sit down, and check the ankle before you decide anything.

Can you walk out?

Try the four-step test. With the injured foot, can you take four steps, even slowly, without severe pain?

If yes, you can walk out slowly. Use a stick, shorten your steps and rest regularly.

If you cannot take four steps, or the bone at the back or the outer edge of the ankle is very tender to press, stay put and call for help. Send your location, and keep warm and dry. Carry a phone, a whistle and a power bank every time.

What should you do in the first days?

Protect the ankle, but keep it moving gently. Cool packs for short spells, light compression and raising the foot can ease swelling. Walk with a stick or crutch if needed, and avoid long standing in the first days.

Do not apply heat in the first days, and avoid strong massage on the swollen ankle. Our guide to the first days after an ankle sprain goes through the steps.

When do you need an X-ray?

Get a check at a clinic or the emergency department if you cannot take four steps afterwards. Do the same if the bones at the sides or back of the ankle are very painful, or the foot looks out of shape. A doctor decides about an X-ray.

Go quickly if the foot is numb, pale or cold.

A clinical practice guideline on lateral ankle sprains, revised in 2021, covers both first-time sprains and chronic ankle instability [1]. Even a first sprain deserves a plan, because a second sprain is more likely.

Why does rehab matter?

A sprained ankle that feels fine at the kitchen counter can still fail on wet clay. After a sprain the ankle’s balance sense is weaker, and the muscles around it need to relearn how to react. Rest alone does not restore that.

Rehab covers range of movement, calf and ankle strength, balance on one leg and finally uneven ground. Read more about ankle sprain and balance training.

How can you prevent a repeat on Ipoh trails?

  • Wear shoes with grip and ankle support, not smooth soles.
  • Use trekking poles on steep or slippery sections, and watch where you place your foot.
  • Slow down on wet clay and roots after rain.
  • Tape or brace the ankle if you have had a sprain, as a physiotherapist advises.
  • Do balance work at home, such as standing on one leg while brushing your teeth.
  • Turn back before you are tired, because tired legs trip.

How does physiotherapy help?

We check the ankle, set a staged plan and show you how to rebuild balance and strength. We also help plan the return to hills, in line with return to walking and hiking. Our sports rehabilitation is available through home visits across Ipoh now.

Our Canning Garden centre, near Fatimah Hospital, is opening soon. Home and centre prices differ and are quoted on WhatsApp.

Questions people ask

Should I keep walking on a sprained ankle?

Stop first and see how it feels. If you can put weight on it and take four steps, you can walk out slowly with a stick, taking care. If you cannot, or the ankle bones are very tender, stay put and call for help.

When do I need an X-ray?

Get one if you cannot take four steps straight after the injury and at the clinic, or if pressing on the back edge of the ankle bone or the bone on the outside of the foot is very painful. A doctor makes the call, usually at an emergency department or clinic.

Do I need to rest completely?

A short rest, yes. Then gentle movement and walking as pain allows usually help recovery. Complete rest for weeks can leave the ankle stiff and unstable. A physiotherapist can show you what is safe at each stage.

Why does my ankle keep giving way?

After a sprain the ligaments can stay loose and the balance reflex weak, which leads to repeat sprains. This is called chronic ankle instability. Balance training and strength work can help, and a brace on uneven ground.
